Product Deficiency Reports

The are two options for submitting Product Deficiency Reports, manual and automated.

Manual Submittal

To manually submit Product Deficiency Reports, use the links below to download Form DASD0038 "Product Deficiency Report" and Field Service Bulletin DASD0039 "Product Deficiency Procedure".

Completed Form DASD0038's can be submitted by Fax, Email, or Postal Mail. Details for these submittal methods can be found in Field Service Bulletin DASD0039.

Automated Submittal

To submit Product Deficiency Reports using the automated system, fill out the form at the bottom of this page to register for a member account. After receiving an account login click on the link below for the "Member Login Page". After logging in click the "Create a new one" link under the Deficiency Report heading.

Using the automated option will speed up the process of transmitting necessary information to Dillon Aero. This option also allows the user access to all submitted Deficiency Reports as well as the Dillon provided corrective action for these reports.
